| bgcolor="#cccccc" | Ondanks de hoge prevalentie van anale dysplasie lijkt screening van HIV-patiënten op anuscarcinoom middels anuscytologie vooralsnog niet zinvol. Er zijn geen gerandomiseerde, prospectieve klinische onderzoeken die een verbeterde overleving van HIV-patiënten laten zien door screening op anuscarcinoom middels anuscytologie. Bovendien is er geen goede standaardbehandeling voorhanden. Verder onderzoek is noodzakelijk naar verbeterde methoden voor preventie, diagnostiek en behandeling van anale dysplasie bij HIV-positieve patiënten. <br>Gezien de snellere progressie en verminderde regressie van cervicale dysplasie en het beschikbaar zijn van een effectieve screeningsmethode en behandeling verdient het aanbeveling alle HIV-positieve vrouwen jaarlijks te screenen middels cervixcytologie onafhankelijk van het CD4-getal.<br>De rol van HPV-diagnostiek als primaire screening op anogenitale maligniteiten is niet duidelijk. De resultaten van enkele grote Europese prospectieve, gerandomiseerde studies worden binnenkort gepubliceerd. In het geval van een “borderline” uitslag bij cervixcytologie wordt aanvullende HPV-diagnostiek aanbevolen volgens de richtlijn van de Nederlandse Vereniging voor Pathologie [NVVP, 2006].<br>Over het penis- en vulvacarcinoom als uiting van anogenitale maligniteiten zijn slechts case-reports verschenen. De werkgroep is van mening dat regelmatig lichamelijk onderzoek in dit geval de beste screeningsmethode is.

Momenteel is er geen indicatie voor routinematige screening op anuscarcinoom. Indien screening toch gewenst wordt geacht, gaat de voorkeur uit naar hoge resolute anoscopie. Voor de screening van mensen met hiv op overige anogenitale maligniteiten wordt verwezen de NIH Guidelines for the Prevention and Treatment of Opportunistic Infections in HIV-Infected Adults and Adolescents (update juni 2019).


Cervix cytologie bij mensen met hiv: start screening en follow-up

Start screening:

  • < 30 jaar: Screening met een PAP-test dient gestart te worden binnen een jaar na de sexarche onafhankelijk van de hiv-transmissiewijze, maar niet later dan 21 jaar. Bij vrouwen tussen de 21 en 30 jaar dient screening bij de hiv-diagnose te geschieden.
  • > 30 jaar: Screening met een PAP-test bij voorkeur met hrHPV-bepaling dient bij hiv-diagnose te geschieden.


Follow-up screening na PAP 1 en hrHPV negatief:

  • < 30 jaar: Jaarlijks cervixcytologie zonder hrHPV-bepailng (dan ook inspectie vulva en anus op VIN of AIN). Indien 3x PAP 1, dan controle elke 3 jaar tot 30e jaar.
  • > 30 jaar: Jaarlijkse cervixcytologie bij voorkeur met hrHPV-bepaling (dan ook inspectie vulva en anus op VIN of AIN). Indien er geen hrHPV-test beschikbaar is: na 3x PAP 1, controle elke 3 jaar. 
    Indien additionele HPV-bepaling: PAP 1 en hrHPV negatief: volgende controle 3 jaar later


Follow-up afwijkende PAP test of hrHPV positief: door gynaecoloog:

  • PAP 1 en hrHPV positief: volgende controle 1 jaar later
  • PAP 2 en hrHPV negatief: volgende controle 1 jaar later
  • Indien PAP 2 en hrHPV positief: controle na 6 maanden herhalen, beleid via gynaecoloog
  • Indien > PAP 2, ongeacht hrHPV status : beleid via gynaecoloog


Periode van screening:

  • In principe levenslang na vaststellen van hiv-diagnose.
  • Bij vrouwen van 65 jaar en ouder kan overwogen worden om te stoppen met de screening als er minimaal 3 keer een PAP 1 werd vastgesteld, hrHPV negatief was en er geen risicofactoren bestaan om hrHPV besmet te raken (expert opinion).

Behandeling van afwijkingen:


Preventie van anuscarcinoom

Momenteel is er geen indicatie voor routinematige screening op anuscarcinoom. Indien screening toch gewenst wordt geacht, gaat de voorkeur uit naar hoge resolute anoscopie. Vergeleken met anale cytologie, zoals in de richtlijn wordt besproken, heeft HR anoscopie een hogere specificiteit en een betere sensitiviteit voor met name hooggradige afwijkingen bij hiv-positieve MSM.[Darragh, 2011; Ho, 2010; Botes, 2013]  . 

Behandeling AIN en anuscarcinoom

Sinds publicatie van de richtlijn is er een drie-armige gerandomiseerde studie gepubliceerd. Deze laat zien dat electrocauterisatie (ECA) superieur is aan imiquimod en topicale 5-fluorouracil (5FU), met een complete response van 50%, 29% en 19% respectievelijk (per-protocol).[Richel, 2013] Hoewel niet significant lijkt imiquimod effectiever bij peri-anale AIN (100% vs 57% [5FU] en 75% [ECA]). Dit wordt ondersteund door een prospectieve enkelarmige studie uit 2006, met een complete response rate van 61% bij patiënten met voornamelijk peri-anale AIN.[Wieland, 2006]
Wij adviseren dan ook electrocauterisatie als eerste behandelkeus voor intra-anale AIN, en imiquimod voor peri-anale AIN.
Gezien de hoge recidief percentages is het aan te bevelen patiënten na behandeling van hooggradige AIN jaarlijks te blijven vervolgen met hoge resolutie anoscopie.


HPV-vaccinatie

  • HPV-vaccinatie wordt vooralsnog niet geadviseerd vanwege gebrek aan bewijs.
